Our service include: - Liaison to families living at a distance -Hospital advocacy -Transition support from hospital/rehab to home -Arrangement, screening and monitoring of in-home care -Crisis intervention and management -Senior housing recommendations to independent, assisted and skilled nursing facilities. -Home safety recommendations (CAPS certified) Unlike "free" placement services, no referral fee is ever accepted. Member of the Aging Life Care Association (formerly National Association for Geriatric Care Managers) and to the ALCA Standards of Practice and Code of Ethics.
