There are multiple medicare related identifications for medicare providers. Medicare UPIN has been replaced by NPI and is no longer used. A PTAN is a Medicare-only number issued to providers by MACs upon enrollment to Medicare - it is also sometimes called Medicare ID or Medicare PIN. An individual provider can have more than one PTAN number but only one NPI Number while hospitals and organizations may have more than one NPI.
