Our focus at Horizon Care Services has always been for seniors to stay at home and safely age in the comfort of their own home comfortably, for as long as possible. However when health problems are a concern, illnesses develop, or accidents happen, an elderly person may need additional home care services or care in a facility. Our goal as care managers is to coordinate needed healthcare and community services to meet the short-term and long-term care of our clients. Without proper planning and coordination of care, unnecessary expenses and institutionalization may occur. Our care managers assist elderly and disabled individuals and their families with short-term and long-term care needs. We strive to empower seniors and their families. Whether family members live near or far, they can be assured that their elderly mother, father, or other loved one is personally respected and cared for properly. History Established in 2004. Established in 2004 Disheartened by the lack of excellence in Florida's health care companies Horizon Care Services put together a plan, opening it's doors in the summer of 2004. Within 1 year through the combined talents and abilities of our team, Horizon Care Services opened 3 branches that are now serving all of South Florida. Currently we are working on expanding with the opening of branches 5 and 6. Horizon Care Services quickly grew to be one of South Florida's premier Home Health Care Providers. We understand the hardship of calling a stranger to care for a loved one, and leave no stone unturned in seeing that everyone concerned finds utmost satisfaction from their service for which they strive to be "Best in the Field".
